Florida’s Medicaid Long-Term Care (LTC) program may help — and many families don’t realize they qualify. This guide walks you through everything you need to know to get support for home care services.


What is Medicaid Long-Term Care (LTC)?

It’s a Florida program that helps seniors and adults with disabilities receive care at home, in assisted living, or in a nursing facility — at little to no cost.


Who Qualifies?

  • Age: 65+ or 18+ with a disability

  • Income limit: Around $2,829/month (2025 est.)

  • Assets limit: Around $2,000 for individuals

  • Health needs: Must require assistance with daily tasks like bathing, dressing, eating, or medication


How to Apply:

  1. Call the Aging and Disability Resource Center (ADRC) at 866-684-5885 to complete a screening.

  2. Wait for a priority score (higher scores get help faster).

  3. Once approved, choose a managed care plan (like Simply, Humana, etc.).

  4. Work with a care manager to select services — that’s where we come in!
